Job Title:Nurse Practitioner or Physician Assistant

Job Description
We are seeking a dedicated Nurse Practitioner (NP) to provide high-quality care in post-acute settings. In this role, you will support patients during their recovery following hospital discharge, helping to reduce readmissions and improve overall outcomes. This position offers a structured weekday schedule, autonomy in practice, and a collaborative team environment.

Key Responsibilities:

  • Conduct rounds in assigned facilities on a consistent weekday daytime schedule.
  • Perform focused patient assessments to monitor recovery, manage pain, wounds, and functional decline.
  • Collaborate with physicians, therapists, and facility staff to develop individualized care plans.
  • Complete documentation using an electronic health record (EHR) system (with remote flexibility).
  • Identify and address readmission risks and gaps in post-acute care.
  • Ensure compliance with applicable clinical and regulatory standards.

Qualifications:

  • Master's degree in Nursing with certification as a Nurse Practitioner (NP) from an accredited program.
  • Active, unrestricted NP license with prescriptive authority in the state of practice.
  • 2-3 years of experience in PM&R, geriatrics, post-acute care, or a related field preferred.
  • Experience working in nursing facilities is a plus.
  • Strong clinical skills in managing recovery, rehabilitation, pain, and chronic conditions.
  • Excellent communication and time-management skills.
  • Proficiency with EHR systems and remote documentation.
  • Ability to work independently with a high level of autonomy.
