As a Senior Transitional Care Nurse Practitioner, you'll practice at the top of your license while helping shape the future of advanced practice providers within one of the region's leading academic health systems. This unique role blends direct patient care, leadership, education, quality improvement, and evidence-based practice to create meaningful change for patients, providers, and the organization.

What You'll Do

Provide Exceptional Patient Care

  • Deliver comprehensive, patient-centered care in collaboration with physicians and interdisciplinary teams.
  • Coordinate seamless transitions of care from admission through discharge.
  • See patients in a Coordinated Care Center, providing steppingstones to successful transition to the ambulatory setting.
  • Educate patients and families on disease management, medications, prevention, and healthy lifestyles.
  • Advocate for patients by addressing medical, social, and behavioral health needs while coordinating specialty referrals and community resources.
  • Facilitate timely, safe discharges that improve patient experience and reduce readmissions.

Drive Quality & Innovation

  • Partner with multidisciplinary teams to improve patient safety, length of stay, cost of care, and patient satisfaction.
  • Help shape organizational policies and innovative models of care that improve healthcare delivery.

What We're Looking For

Required Qualifications

  • Master of Science in Nursing with completion of an accredited Nurse Practitioner program.
  • Current Maryland Nurse Practitioner license and national certification in specialty area.
  • DEA and CDS eligibility.
  • ACLS/BLS certification (as applicable).
  • Minimum of three years of Nurse Practitioner experience.

Work Experience
