Location: Ogden, Utah
Schedule: 12 hours per week (600 hours annually)
Population: Students ages 16-24Primary Responsibilities
- Conduct physical examinations and develop treatment plans.
- Diagnose and treat acute and chronic medical conditions.
- Prescribe medications within scope of practice and supervisory authority.
- Administer immunizations and preventive health services.
- Document all patient encounters accurately and timely.
- Participate in DAC meetings when requested.
- Coordinate referrals for specialized or emergency care.
- Collaborate with behavioral health and substance abuse staff.
- Maintain infection control and HIPAA compliance standards.
Required Qualifications
- Active, unrestricted Utah Physician Assistant license.
- Valid DEA registration with prescribing authority.
- Current professional liability insurance of at least $1,000,000 per occurrence.
- Ability to pass required background screening and drug testing.
- Experience in primary care; experience with adolescent/young adult populations preferred.
- Ability to work both independently and under physician supervision as required by state law.
Supervisory Requirement
- Must maintain a current Supervisory Agreement with a licensed Physician in accordance with Utah regulations.
- Supervisory Agreement must support prescribing authority, including controlled substances.
- All licenses and supervisory agreements must remain active during the full contract performance period
