Join our brand-new High Risk Clinic at Berkshire Health Systems in Pittsfield, MA, as a Physician Assistant or Nurse Practitioner. This role offers significant autonomy and the chance to help design a program focused on truly comprehensive care for patients facing complex, everyday challenges.
Key Responsibilities
- Conduct in-depth health assessments and develop individualized care plans
- Provide holistic medical care, addressing physical, mental, and social needs
- Coordinate with Behavioral Specialists, Care Coordinators, and nursing staff
- Assist patients with non-medical needs such as securing housing, employment, or transportation
- Maintain patient engagement without volume-based pressure-no set daily patient quota
Qualifications
- Minimum of 3 years of clinical experience, including primary care
- Experience in integrated care or harm reduction models preferred
- Comfort working with high-risk populations (PTSD, severe anxiety, SUD, disabilities)
- Strong collaboration, communication, and problem-solving skills
