Location: 5 Boroughs of NYC

Employment Type: Flexible, Per Diem

Compensation: $170/hour

We are seeking a compassionate and experienced Nurse Practitioner (NP) to provide high-quality care for medically fragile patients under the Home and Community-Based Services (HCBS) program. This role focuses on delivering patient-centered care, including pain and symptom management, with an emphasis on improving quality of life.

Key Responsibilities:

  • Provide comprehensive assessments and ongoing care for medically fragile patients
  • Deliver pain and symptom management services in home/community settings
  • Collaborate with interdisciplinary teams to develop and implement care plans
  • Support patients and families, including those requiring end-of-life care
  • Ensure compliance with all HCBS and New York State regulations

Requirements:

  • Active Nurse Practitioner license in New York
  • Certification or qualification in Pain and Symptom Management
  • Minimum 3 years of experience working with medically fragile populations
  • At least 1 year of clinical experience with pediatric patients
  • Minimum of 3,600 clinical hours as a Nurse Practitioner
  • Experience involving end-of-life care strongly preferred
  • Strong clinical, communication, and documentation skills
