Join a collaborative, patient-focused clinical team delivering comprehensive acute and chronic care management for adult and geriatric patients in a rehabilitation hospital. You'll partner closely with supervising physicians and interdisciplinary care teams to drive real improvements in patient outcomes and quality of life, all on a predictable Monday-Friday, 7am-5pm schedule in Oak Creek, WI.

What You'll Do
- Provide primary care for adult and geriatric patients with acute and chronic conditions
- Perform comprehensive assessments, establish diagnoses, and develop individualized treatment plans
- Prescribe medications and order/interpret diagnostic tests within your scope of practice
- Coordinate care and clinical decision-making with facility teams and supervising physicians
- Maintain accurate, timely, and thorough clinical documentation
- Collaborate with specialty providers (e.g., wound care, psychiatry) for integrated care delivery
- Communicate clearly with patients, families, and care teams on care plans and changes in condition
- Build trusted, lasting relationships with patients, families, and facility staff
What You Bring
- Active Nurse Practitioner or Physician Assistant license in good standing
- Active DEA registration
- Current BLS certification
- Strong diagnostic and clinical reasoning skills
- Ability to work independently in a structured care environment
- Strong collaboration and communication skills
- Commitment to compassionate, patient-centered care
