Position Description

Home clinic opportunity would be average 32-33 hours/week over a 2 week period and every other weekend (a traditional schedule)

Roles Responsibilities
• Provide Primary and Family evidenced-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients.
• Provide administrative duties to include but not limited to opening/closing the clinic, inventory, patient follow up, verifying insurance and collecting payment, maintaining a clean environment.
• Be responsible for handling patient appointments and performing routine examinations to ensure positive patient health outcomes.
• Perform duties including speaking with patients about healthy lifestyle choices, completing diagnostic tests to help diagnose and treat patients for injuries or illnesses and coordinating with other healthcare professionals to maintain a comprehensive plan of care.
• Encourage patients to become active participants in their own healthcare and well-being through services such as health screenings, behavioral health and wellness, chronic condition management.

Skills
• Candidates must have prior primary care or urgent care experience.
• Completion of Physician Assistant program with current National Board Certification and State of Employment license to practice.
• A minimum of 1 year of recent Primary Care experience as a PA in either a family medicine or primary care practice.
• Working knowledge and understanding of quality measurement in the management of chronic disease conditions.
• Ability to work remotely in a clinical care team culture.
• Ability to work without direct supervision and practice autonomously.

Education
• Completion of a: Masters of Science in Physician Assistant Studies (or other health-related Masters Degree in conjunction with Bachelors of Science in Physician Assistant Studies) with current National Board Certification and State of Employment license to practice in the role required.
