Position Summary

As a Palliative Care Nurse Practitioner (NP) with Impact Health PC in Cleveland, OH, you will provide compassionate, patient-centered care to individuals with serious and complex illnesses within skilled nursing facilities. You will assess symptoms, develop individualized treatment plans, support goals-of-care discussions, and collaborate closely with patients, families, facility staff, physicians, and interdisciplinary care teams.
Key Responsibilities

• Provide compassionate, person-centered palliative care using evidence-based practices.
• Conduct comprehensive assessments and develop individualized treatment plans.
• Manage symptoms and provide ongoing follow-up for patients with serious and complex illnesses.
• Support goals-of-care discussions with patients and families.
• Collaborate with physicians, facility staff, and interdisciplinary care teams.
• Educate patients, families, and facility staff regarding palliative care and treatment plans.
• Complete timely and accurate clinical documentation.
• Maintain compliance with facility, state, and federal regulations.

• Minimum 2 years of post-certification experience preferred.
