Work Schedule: Days with Call Weekend and Holiday Rotation

Hours: 8:00am to 4:00pm

Summary:
The Nurse Practitioner (NP) functions as a member of an inter-professional team to provide comprehensive health assessment, diagnosis and treatment of individuals with acute, sub-acute and advanced illness. NP provides medical care in support of professional medical staff. Assists in establishing and monitoring standards of care, provides expert clinical intervention related to symptom management, discussion of goals of care with patients and their families and provides leadership in standards and research utilization in the nursing care environment. Provides a variety of medical services to patients in accordance with established laws and procedures. Works cooperatively with other health professionals under the direct supervision of a physician.

Responsibilities:

EDUCATION

  • Graduate[MCL1] from an accredited school of professional nursing required
  • NYS Licensed Registered Nurse
  • Certified as a Nurse Practitioner by NYS
  • Meets credentialing requirements as appropriate through the Medical Staff office required
  • Board Certified by either AACN or AANP required

EXPERIENCE

  • Three (3) years experience as a Nurse Practitioner preferred
  • Advanced content knowledge in palliative care and hospice
  • Certification in CPR and annual re-certification required
  • ACLS certification preferred

KNOWLEDGE, SKILLS AND ABILITIES

  • Strong Written and verbal communication skills
  • Strong organizational skills
  • Computer skills
  • Knowledge of Home Care and Hospice services
  • Knowledge of community resources

WORKING CONDITIONS

  • Frequent sitting, standing and walking
  • Visual and auditory acuity are essential
  • Works in office setting and patient care units throughout facility
  • Exposure to infectious diseases and uses appropriate personal protective equipment
  • Working hours are varied, works beyond normal working hours, weekend coverage as necessary
