Work Shifts

3:00 P.M - 11:00 P.M (Alternating Work Schedule)

Duties & Responsibilities

The HEAL Program is a new NYC H+H initiative to transition into being the primary provider of medical services to young people in city custody. NYC H+H employed providers will be responsible for 24/7 medical care across two secure detention sites, two school-based health centers, and limited pharmacy services at seven non-secure sites (group homes). Services include comprehensive medical screenings, dental screenings and procedures, vaccinations and routine testing, responses to immediate need, and referrals for offsite care within and outside the NYC H+H system.

SUMMARY OF ESSENTIAL DUTIES AND RESPONSIBILITIES:

Under the guidance of the Medical Director for the HEAL Program, the Nurse Practitioner will perform the following duties:

Patient Care:

  • Provide medical care to ACS adolescents keeping safety & security measures paramount.
  • Take relevant patient history.
  • Identify patient needs and makes diagnoses.
  • Develop and implement treatment plans using diagnostic data.
  • Perform complete well-care examinations including physical examinations.
  • Perform complete gynecological histories and physical examinations as indicated.
  • Write prescriptions and inform patients about medications prescribed; garner parent/guardian consents when needed.
  • Complete medical forms and documentation related to patient care.
  • Make and track all referrals to other agencies and providers, with follow-up care provided.
  • Assist with phlebotomy, as needed, to ensure smooth patient flow.
  • Administer immunizations as needed.
  • Distribute medication to patients.
  • Review all pertinent laboratory results and makes appropriate treatment decisions based on these data.
  • Complete Court Orders and all other ACS mandated paperwork.
  • Work collaboratively with physician and specialists for consultation and referral purposes.
  • Evaluate and treat ACS employees [ONLY on an emergent basis].
  • Administer vaccinations to H+H/ACS employees based on protocol.
  • Coordinate organization of prescription medications involving ordering refills for secure and non-secure detainees, discarding medications that are discontinued and preparing medications for NSD.
  • Adhere to clinical guidelines provided by the Medical Director.
  • Meet with designated supervising physician on a regular and predetermined basis, in order to receive recommendations and collaborate towards delivering quality health care to patients, based on accepted standards of-care.
  • Participate in Peer Review process, as defined by NYC H+H.
  • Encourage health promotion, disease prevention, and health maintenance; provide health education on topics including but not limited to preventive medicine and counseling on chronic diseases, as well as parenting, prenatal, nutrition, family- planning, medication usage, and stress management.
  • Provide patient advocacy, as appropriate for the site.
  • Participate in patient outreach, as appropriate for the site
  • Other duties as assigned

Administrative Duties:

  • Adhere to NYC H+H and ACS Policies and Procedures
  • When in the field, act as on-site supervisor of Patient Care Associates.
  • Teach PCAs about patient care and triage, to improve patient flow, and communicate with Director of Nursing regarding status of flow at clinic site.
  • Participate in case conferences in which patient care is discussed.
  • Assist the Medical Director in the development and implementation of medical protocols and medical forms.
  • May initiate and participate in research related to health care or allied fields, in collaboration with the Medical Director.
  • Make suggestions to clinic supervisors to improve patient care.

ASSIGNMENT, REVIEW AND APPROVAL OF WORK

  • The Medical Director may assign other duties and projects as needed.

Minimum Qualifications
1. A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
2. Current certificate as a Nurse Practitioner issued by the NYSED; and
3. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).

Department Preferences

Please note this position will be rotated between facilities between Brooklyn and the Bronx.

  4. Strong verbal and written communication skills
  5. Ability to de-escalate difficult situations in a calm and professional manner
  6. Ability to build rapport with patients, families, and colleague

Additional Salary Compensation

In addition to base compensation, you may also receive the following based on the Collectively-Bargained Agreement:

An annual education differential if you currently possess an appropriate degree in nursing or an allied health discipline from an accredited college or university, as listed below:

  • Bachelors of Science in Nursing (BSN) or Bachelors in allied health discipline = $900
  • Masters of Science in Nursing (MSN) or Masters in allied health discipline = $1,200
  • Doctorate in Education, Nursing Practice or PhD in Nursing (DNP) = $2,500

An annual certification differential of $1,878 if you currently hold an appropriate certification that is nationally recognized in the discipline or area of specialization for which you are employed.

An annual nursing experience differential ranging from $1,097 up to $9,100 for up to 10 years of experience if you have outside nursing experience as a Nurse Practitioner.
