Nurse Practitioner - Family Medicine (Full-Time)
The Nurse Practitioner provides continuing and holistic healthcare service to individuals and families.
Essential Duties and Responsibilities:
- Delivers a variety of healthcare services, including preventative, chronic and acute services.
- Performs diagnosis and treatment of illnesses, carries out health-risk assessments, routine check-ups, screening tests and immunizations, and counseling to individuals on ways to maintain a healthy lifestyle, and to prevent diseases.
- Ensure participation in continuing education and training to keep abreast of current developments in the industry.
- Provide quality care to patients by following the most current evidence-based guidelines and procedure of practice.
- Takes part in quality assurance audits.
REQUIREMENTS
Nurse Practitioner License in State of Illinois
Current Illinois Controlled Substance License within 6 months of hire
Eligible for DEA registration Possession of a current DEA Registration within six months of hire
Current ACLS and BLS certification
• Licensed and certified as a Nurse Practitioner (NP) in the state of Illinois
• Board-certified in Family Medicine, Adult-Gerontology, or related field Prior experience in primary care
• Strong clinical assessment, communication, and documentation skills
• Commitment to providing high-quality, compassionate care
Bilingual IN Spanish strongly preferred
