What will I do as an employee with Redstone?
As the Nurse Practitioner (CRNP) - Hospice, supporting the Redstone mission will include the following responsibilities:
- Conduct face-to-face hospice visits to assess and document patients' continued eligibility for the Medicare Hospice Benefit.
- Perform comprehensive patient assessments and provide palliative care interventions to support comfort and quality of life.
- Collaborate with the Hospice Medical Director, attending physicians, and interdisciplinary team to coordinate individualized patient care.
- Document patient visits, clinical findings, and required hospice recertification information accurately and timely.
- Provide consultation and clinical support regarding palliative and end-of-life care while maintaining compliance with Medicare and regulatory requirements.
- Promote compassionate, patient-centered care while supporting the mission and philosophy of Redstone Hospice.
What do I need for this role with Redstone?
- Current Pennsylvania Certified Registered Nurse Practitioner (CRNP) license and national certification.
- Masters degree or higher from an accredited Nurse Practitioner program.
- Minimum of 2 years of Nurse Practitioner Experience
- Understanding of hospice and palliative care principles preferred.
- Must possess a valid Pennsylvania driver's license and reliable transportation.
