Nurse Practitioner - Adult OP Mental Health Services

Middlesex Health is seeking a compassionate and skilled Psychiatric Advanced Practice Registered Nurse (APRN) to join our expanding Adult Outpatient Mental Health Services team. This (35 hours/week) position offers the opportunity to provide high-quality psychiatric care within a collaborative, multidisciplinary environment dedicated to delivering exceptional patient outcomes.

As a Psychiatric APRN, you will have the unique opportunity to support continuity of care across the mental health services continuum. You will be part of a highly supportive clinical and administrative team committed to excellence in patient care and professional growth.

Responsibilities

  • Conduct psychiatric evaluations and diagnostic assessments for adult patients
  • Develop and implement individualized treatment plans
  • Prescribe and manage psychiatric medications in accordance with state and federal regulations
  • Provide ongoing medication management and psychiatric follow-up care
  • Collaborate with therapists, social workers, primary care providers, and other members of the treatment team
  • Maintain timely and accurate clinical documentation within the electronic health record
  • Participate in quality improvement initiatives and support a patient-centered model of care

Qualifications

  • Graduate of an accredited school of nursing
  • Current Connecticut APRN license
  • Board certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP) or eligibility to obtain certification
  • Experience working with mental health populations in outpatient and/or inpatient settings
  • Strong interpersonal, communication, and clinical assessment skills
  • Commitment to providing outstanding patient care and promoting a collaborative work environment
