Title: Nurse Practitioner - Float | Kansas City Metro

Summary:
Our client is seeking an experienced Family Nurse Practitioner to join a growing outpatient primary care organization dedicated to serving the senior population through a value-based care model. This float position will provide coverage across multiple Kansas City-area clinics while working alongside physicians and an interdisciplinary care team. Providers enjoy longer appointment times, smaller patient panels, and the opportunity to build meaningful relationships with patients while focusing on preventative care and improving long-term health outcomes.

Day to Day:

  • Provide comprehensive outpatient primary care to adults ages 65+
  • Diagnose, treat, and prescribe medications for acute and chronic conditions
  • Collaborate with physicians, care managers, and clinical staff to coordinate patient care
  • Perform comprehensive assessments and develop individualized care plans
  • Focus on preventative care, chronic disease management, and reducing unnecessary hospital and ER visits
  • Complete patient documentation and follow-up using the electronic medical record system
  • Conduct telehealth visits or administrative work when patient schedules allow

Shift: Monday - Friday | 8:00 AM - 5:00 PM

Typical daily schedule:

  • 8:00 AM - Team huddle
  • 8:30 AM - First patient
  • 12:00 PM - 1:00 PM - Lunch
  • 3:30 PM - Last patient
  • End-of-day administrative time

Qualifications:

  • Active Family Nurse Practitioner (FNP) license (or ability to obtain applicable state licensure)
  • Current BLS or ACLS certification
  • Minimum of 2 years of Nurse Practitioner experience
  • Background in Family Medicine, Internal Medicine, Primary Care, or Geriatrics
  • Experience working with geriatric patients
  • Value-based care, managed care, or Medicare Advantage experience preferred
  • Experience with eClinicalWorks EMR is a plus (training provided)
  • Compassionate, patient-focused provider with strong communication and collaboration skills
