JOB DESCRIPTION
NURSE PRACTITIONER
Reports to: Medical Director for Clinical Supervision
 Chief Operations Officer for Administrative Supervision
Salary: Negotiable
Duties and Responsibilities:
<ol>
<li>Evaluation and diagnosis of consumers in outpatient setting</li>
<li>Will conduct routine medication management and six-month reassessments</li>
<li>Pharmacological treatment for patients including prescribing medications and providing follow -up in office or in the field if needed</li>
<li>Provide continuity of care by facilitating hospital admits when needed</li>
<li>Coordinate care with other service providers inside and/or outside of the agency. This includes availability for telephone consultations and team meetings as needed</li>
<li>Consult with the supervising physician as needed</li>
<li>Responsible for ensuring that 20% of work is reviewed by the Medical Director or designee every 30 days as required by state licensing board</li>
<li>Other duties as assigned</li>
</ol>
Requirements:
<ul>
<li>RN degree; Advance Practice Nurse</li>
<li>Full unrestricted TN license or multistate license that includes TN</li>
<li>Nurse Practitioner certification; Psychiatry certification preferred</li>
<li>Previous mental health experience</li>
<li>Excellent communication skills</li>
