We are seeking a dedicated and compassionate full-time Family Nurse Practitioner to join our multidisciplinary team in providing professional medical services for AACI Health Center patients.
As Part of Our Team, You Will:
- Provide comprehensive primary care services, including assessment, screenings, diagnosis, treatment, and preventive health services.
- Conduct physical examinations, obtain medical histories, and develop appropriate treatment plans.
- Prescribe and manage medications, develop individualized treatment plans, and monitor patient outcomes in collaboration with the care team
- Educate patients on disease prevention, medication adherence, and overall wellness.
- Collaborate with physicians, behavioral health providers, and interdisciplinary teams to ensure coordinated, high-quality care
- Maintain accurate, timely, and complete documentation in the electronic health record (EHR).
- Coordinate referrals and consultations with specialists to support comprehensive patient care
- Support team-based operations, including meetings, cross-coverage for clinicians, and participation in after-hours on-call rotation
Qualifications
- Master's Degree or Doctorate from an accredited Nurse Practitioner program
- Current, unrestricted License as a Nurse Practitioner in the State of California
- Board Certification from the California Board of Registered Nursing
- Active and unrestricted DEA license
- Current BLS certification
- Minimum 2 years of Primary Care clinical work experience required
- Experience working in a community health center, FQHC, or underserved healthcare setting preferred.
