How You'll Make an Impact
As a Mobile Nurse Practitioner, you'll bring high-quality primary care services directly to patients in their homes and virtually when appropriate.
You will:
- Conduct comprehensive health assessments and follow-up visits
- Diagnose and manage acute and chronic health conditions
- Develop and implement personalized care plans
- Order, interpret, and evaluate diagnostic tests and laboratory results
- Prescribe medications and other treatments in accordance with state regulations
- Educate patients and caregivers on disease prevention and healthy lifestyle choices
- Collaborate with interdisciplinary care teams to coordinate care and improve outcomes
- Identify barriers to care and help connect patients with resources and support services
- Participate in on-call responsibilities as needed
Service Areas
This field-based role supports patients throughout San Gabriel Valley & East LA. Regular travel throughout the assigned territory is required.
What We're Looking For
Minimum Qualifications
- Master of Science in Nursing (MSN)
- Active, unrestricted Registered Nurse (RN) license
- Active, unrestricted Nurse Practitioner (NP) license
- Minimum of one year of Nurse Practitioner clinical experience
- Strong assessment, diagnostic, and care management skills
- Valid driver's license and reliable transportation
- Ability to work independently and collaboratively within a multidisciplinary team
- Completion of required TB screening
Preferred Qualifications
- Primary care, family medicine, internal medicine, geriatrics, home-based care, or value-based care experience
- Experience caring for Medicare and medically complex patient populations
- Bilingual Spanish-English skills preferred

$119,313.00
to
$178,968.00
The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
