<li>Perform cognitive screenings, annual wellness visits, follow-ups, and hospital-to-home transition supervision</li>
</ul>
What You Bring:

<li>Willingness to travel to patient's homes</li>
<li>Active Virginia Nurse Practitioner</li>
<li>DEA registration or eligibility</li>
<li>Medicare PECOS enrolled</li>
<li>Experience in primary care and geriatrics</li>
</ul>
