About the Role We are seeking a skilled and compassionate Nurse Practitioner to perform required Face-to-Face (F2F) encounters for hospice patients approaching benefit period recertification.
Key Responsibilities
- Conduct Face-to-Face encounters for patients entering their 3rd benefit period and beyond.
- Assess and document terminal prognosis, disease progression, functional decline, and symptom burden.
- Collaborate with hospice physicians, RNs, social workers, and the IDG to support accurate recertification decisions.
- Provide clinical insight and education to staff regarding disease trajectories, eligibility criteria, and hospice best practices.
- Maintain timely, compliant documentation aligned with Medicare and state regulatory standards.
- Communicate findings effectively to physicians and clinical leadership.
- Travel within assigned New Mexico service area to visit patients in homes, assisted living facilities, SNFs, or other care settings.
Qualifications
- Current and unrestricted Nurse Practitioner license in New Mexico.
- National board certification as NP (FNP, AGNP, or similar).
- Minimum 1-year clinical experience; hospice, palliative care, or geriatric experience strongly preferred.
- Solid understanding of hospice eligibility criteria and disease trajectories.
- Strong communication, interpersonal, and clinical assessment skills.
- Reliable transportation and willingness to travel across assigned territory.
