POSITION SUMMARY:
Under the direction of the Clinic Coordinator, the Nurse Practitioner (NP) or Physician Assistant (PA) works collaboratively within a multidisciplinary health team, is responsible for providing comprehensive care to patients with acute or chronic medical conditions in an ambulatory and school-based setting that includes telehealth medicine. He/she demonstrates a high degree of clinical expertise in working with patients with acute and chronic illnesses or acute injuries commonly encountered in both the pediatric and adult patient population. He/she is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination for patients and families with acute and chronic health needs. The NP or PA demonstrates an advanced level of medical clinical knowledge, communication and interdisciplinary collaboration, professionalism and systems-based practice. He/she works with administrative, nursing, physician, and BMHC staff to ensure safe, effective, quality patient care and to promote BMHC mission. The provider practices within their professional scope, licensure, certification, credentialing and clinical privileges and follows applicable federal, state, tribal, and BMHC requirements and policies.
ESSENTIAL FUNCTIONS:
1. Provides comprehensive preventative and therapeutic health care service to patients in both a community based primary care facility as well as a school-based clinic to serve the primary health resources at both locations.
2. School based healthcare responsibilities including working with parents/guardians, consent and confidentiality, coordination with school personnel when appropriate, and managing/referring students whose needs are beyond what can be handled in the school clinic.
3. Interview and examine patients using the Electronic Health Record (hereinafter termed the "EHR"), review past medical histories and request diagnostic tests necessary to obtain all possible information relevant to each patient's case and care.
4. Make preliminary diagnosis, direct, prescribe and provide treatment, or arrange for specialized care through referrals of patients examined, if determined appropriate. For those patients not requiring referral, to provide definitive management of their cases and care.
5. Provide urgent care as required.
6. Provide patient care in patient's home if needed.
7. Provide telehealth if needed, including evaluation, treatment, follow-up, and documentation through telehealth.
8. Modify treatment regimens as required, and review patient's medications, diet, therapy course, and adherence to applicable management plan. Obtain patients observation of treatment course, and to ensure patients are instructed and counseled on the patient's condition and medical regimen.
9. Responsible for reviewing and following up on labs, imaging, and other diagnostic results, including communicating results to patients and making sure appropriate follow-up occurs.
10. Complete all reports required by the Bay Mills Health Center and participate in program committees and quality assurance activities. Documentation expectations for timely completion of EHR documentation, orders, medication reconciliation, coding/billing documentation, etc.
11. Provide assistance to other medical and administrative staff in areas regarding technical expertise in medical areas.
12. Must demonstrate positive communication through friendly, pleasant, and professional verbal skills. Demonstrates the ability to meet the needs of the patient in a caring, competent manner.
13. Initiates evidence-based care planning with the patient/family and care team.
14. Evaluates patient's response to care through observation and motivational interviewing techniques.
15. Makes/tracks referrals to community-based resources.
16. Manages care of patients through collaboration with members of the health care team and other health professionals in order to make decisions about treatment and assist the patient in attaining health goals.
17. Monitors evidence-based clinical indicators to assess progress toward better health and shares these with the patient/family.
18. Completes community based events and trainings
19. Must attend all mandatory or departmental training offered by the Bay Mills Human Resource Department and/or Bay Mills Health Center.
20. Other duties may be assigned within the scope and complexity of this position's essential functions.
Requirements
PHYSICAL REQUIREMENTS:
While performing the duties of this job, the employee is regularly required to sit, stand, and walk. The employee may be required to lift patients, or other strenuous physical activity when rendering first aid or while providing patient care. The employee completes their work under normal office conditions, temperatures, and is environmentally controlled the majority of the time.
POSITION REQUIREMENTS:
- Graduation from accredited nurse practitioner or physician assistant program required.
2. Michigan license to practice as an Advanced Practice Registered Nurse or Physician Assistant required.
3. Three years' experience working as a practitioner in a primary care setting preferred.
4. Culturally sensitive and responsive care preferred.
5. Experience in working with the school-aged population preferred.
6. Familiarity with privacy and confidentiality regulations and implementation required.
7. Successful candidates must pass a pre-employment drug test and background check as a condition of employment. Candidates are subject to random drug testing as a condition of continued employment.
