General Statement of Duties
Provides direct patient care for the Health Department's STD/HIV clinic. Reports to the Director of Health.
This is a Grant-funded position that will be scheduled to work approximately 4 to 6 hours per week.
Qualifications
Education and Experience:

Master's degree in Nursing, Family Nurse Practitioner (FNP) certification, plus three (3) years of clinical experience. Public Health Nursing experience preferred.
Licenses and Certifications:

Valid CT State Registered Nurse License.

Valid Family Nurse Practitioner (FNP) certification.

Valid CT State Advanced Practice Registered Nurse License and Independent Practice status under Section 20-87a(b)(3) Connecticut General Statutes.

Valid Drug Enforcement Administration (DEA) License.

Basic Life Support (BLS) Certification.

Qualifications:
Demonstrated knowledge of public health practices and procedures, including maternal and children's health.

Demonstrated knowledge and care of communicable diseases.

Demonstrated knowledge of medications and prescriptions, as well as the ability to prescribe appropriate medications.

Demonstrated proficiency in performing physical examinations.

Demonstrated knowledge of the Advisory Committee of Immunization Practices (ACIP) recommended immunization schedules.

Proven ability to maintain records.

Demonstrated proficiency in the use of Microsoft Office products, including Word, Excel, and PowerPoint, as well as industry-specific software.

Proven ability to execute sound, independent judgment in accordance with laws, ordinances, and regulations.

Demonstrated ability to communicate effectively verbally and in writing.

Proven ability to establish and maintain effective professional working relationships with medical/nursing staff, volunteers, and patients.

Duties and Responsibilities:

Develops treatment plans and provides patient health care services in accordance with the STD/HIV clinic's policies and procedures.

Performs comprehensive physical assessments.

Orders laboratory tests and other related procedures as indicated.

Provides treatment, prescribes medication, and educates patients on preventative care options.

Refers patients to specialists for further care based on examinations and laboratory results.

Educates patients with communicable diseases, their partners and close contacts about the condition in order to facilitate cure and prevent contagion.

Maintains confidential records and statistical data for reporting, analysis, and trend identification.

Assists the Director of Special Clinical Services in planning, developing, and implementing changes to public health programs.

Collaborates with other health departments and the Connecticut Department of Public Health to promote the Town's public health programs in the community.

Supports Town and Department of Health policies and philosophies.

Performs other related work as required.
