Salary: $115,000-$128,000 based upon experience

About the Opportunity

Our client, a respected regional healthcare organization, is seeking two Family Nurse Practitioners to join its outpatient primary care team. This position offers the opportunity to build long-term patient relationships while practicing in a collaborative, community-focused environment supported by a larger integrated healthcare network.

Key Responsibilities

  • Provide comprehensive primary care services across all stages of life in an outpatient setting.
  • Diagnose, treat, and manage acute and chronic health conditions.
  • Perform preventive care, wellness visits, patient education, and chronic disease management.
  • Build lasting relationships with patients and families through continuity of care.
  • Collaborate with physicians, advanced practice providers, nursing staff, and interdisciplinary teams.
  • Maintain accurate clinical documentation using the electronic medical record (EMR).
  • Participate in quality improvement initiatives and promote evidence-based care.

Minimum Qualifications

  • Licensed Family Nurse Practitioner (FNP).
  • Active RN license or eligibility for RN licensure in the State of New York.
  • At least one year of experience is preferred in primary care, family medicine, internal medicine, or community health.
  • Strong patient-centered approach with excellent communication and clinical decision-making skills.
