Job Overview
We are seeking a qualified and compassionate Family Nurse Practitioner (FNP) to join an interdisciplinary healthcare team within an Article 28 Primary Care Clinic. This position offers an opportunity to provide high-quality, person-centered primary care to individuals with intellectual and developmental disabilities (IDD).
The ideal candidate will be committed to improving healthcare access, promoting health equity, and delivering comprehensive care tailored to the unique needs of this specialized population.
Key Responsibilities:
- Provide comprehensive primary and preventive care across the lifespan within an Article 28 framework.
- Conduct patient assessments, diagnose medical conditions, and develop evidence-based treatment plans.
- Collaborate with physicians, healthcare providers, and interdisciplinary teams to coordinate individualized patient care.
- Support health promotion, patient education, and chronic disease management, with care tailored to individuals with IDD.
- Participate in quality improvement initiatives and contribute to enhancing clinical workflows, protocols, and patient care practices.
- Help promote health equity and reduce barriers to healthcare access.
- Provide clinical leadership and mentorship to nursing and support staff.
- Maintain accurate and timely clinical documentation in accordance with regulatory requirements and organizational policies.
- Deliver patient services both in person and through telehealth, as appropriate.
- Perform other duties as assigned.
Qualifications & Requirements:
- Current Family Nurse Practitioner (FNP) license and certification.
- MSN or DNP required.
- Minimum of 3 years of clinical experience in primary care, family medicine, or internal medicine.
- Experience working with individuals with intellectual and developmental disabilities is preferred but not required; training and support will be provided.
- Strong clinical assessment, communication, and interpersonal skills.
- Ability to work effectively within an interdisciplinary healthcare team.
- Commitment to inclusive, equitable, accessible, and person-centered healthcare.
- Willingness to obtain required IDD-specific certifications and complete relevant training.
- Strong professional verbal and written communication skills.
- Ability to meet applicable requirements for driving organizational vehicles, as required.
