Position Responsibilities

• Provide comprehensive outpatient Family Medicine care to patients across the lifespan.
• Manage acute illnesses, chronic conditions, preventive care, wellness visits, and routine follow-up.
• Establish and maintain an ongoing patient panel.
• Perform histories, physical examinations, assessments, diagnosis, and treatment planning.
• Order and interpret diagnostic testing and coordinate referrals when appropriate.
• Provide patient education, counseling, and shared decision-making.
• Collaborate with physicians, APPs, specialists, nursing staff, and ancillary services.
• Participate in quality improvement and other clinical initiatives.
• Deliver care consistent with professional standards, organizational policies, and applicable regulations.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Minimum of one year of post-education clinical experience required.
• Experience in Family Medicine, primary care, or a closely related outpatient setting preferred.
• Current board certification through an appropriate national certifying organization.
• Licensed or ability to obtain license as an NP or PA in Minnesota.
• Licensed or ability to obtain DEA licensure.

• Salary range: $96,000 - $145,000
• Production Pay Eligible
