Compensation & Incentives
W-2 Employment
- Base Compensation: $316,800 annually ($180/hour)
1099 Independent Contractor
- Base Compensation: $352,800 annually ($215/hour)
Additional Incentives
- $50,000 Sign-On Bonus (5-year commitment required)
- Eligible for quality incentive of up to $8,800 annually

Qualifications
- Certified Registered Nurse Anesthetist (CRNA)
- Certification through the National Board of Certification and Recertification for Nurse Anesthetists (NBCRNA)
- Active Indiana CRNA license required
