Certified Nurse Practitioner

The NP at PCHC provides comprehensive, patient-centered medical care with a strong focus on Pediatrics with an opportunity of minimal Family Practice, if desired for continuity of care, within a community health center setting.
Key Responsibilities
- Provide primary and preventive care to pediatric patients (birth through adolescence)
- Perform well-child exams, developmental screenings, and immunizations
- Diagnose and treat acute illnesses and chronic conditions
- Deliver family-centered care, education, and anticipatory guidance
- Collaborate with physicians, behavioral health, dental, and care coordination teams
- Participate in quality improvement initiatives and care team meetings
- Maintain accurate and timely medical documentation in accordance with clinical standards
- Adhere to all PCHC clinical protocols and regulatory requirements
Qualifications
- Graduate of an accredited Nurse Practitioner program
- National certification as a Family NP or Pediatric NP (PNP)
- Licensed or eligible for licensure in the State of Colorado
- Current BLS/CPR certification
- 2 years experience in pediatrics, family medicine, or community health preferred
- Strong commitment to serving underserved populations
