Fall River Health Services (FRHS) is seeking a dedicated and compassionate Advanced Practice Provider (APP) to provide family-focused primary care to individuals and families throughout the Black Hills community.

In this role, you will evaluate, diagnose, and treat patients of all ages while promoting disease prevention, wellness, and long-term health. You will manage a variety of acute and chronic conditions, educate patients on maintaining their health, and collaborate with physicians and other healthcare professionals to deliver high-quality, patient-centered care.

Key Responsibilities

• Provide Primary Care Services: Deliver high-quality primary clinical care in accordance with your licensure, scope of practice, and applicable supervision or collaboration requirements. Work closely with clinic staff and other healthcare professionals to maintain clinical standards.
• Conduct Patient Assessments: Evaluate patients of all ages by obtaining comprehensive medical histories, performing physical examinations, reviewing diagnostic studies, assessing psychosocial needs, and maintaining problem-oriented medical records.
• Promote Health and Wellness: Provide patient and family education, counseling, and interventions focused on disease prevention, health maintenance, and restoration of optimal health.
• Diagnose and Treat Acute Conditions: Assess and manage common acute and undiagnosed health concerns using established clinical protocols and sound clinical judgment. Develop appropriate treatment plans and make referrals when indicated.
• Manage Chronic Conditions: Provide ongoing assessment, treatment, and follow-up care for patients with common chronic health conditions. Monitor patient progress and adjust management plans as appropriate.
• Maintain Accurate Documentation: Complete patient records accurately, thoroughly, and promptly while protecting confidential information in accordance with HIPAA, medical staff bylaws, and applicable professional regulations.
• Support Revenue Cycle Processes: Collaborate with facility and clinic staff to support accurate documentation, billing processes, and timely reimbursement for services provided.
• Collaborate with Healthcare Professionals: Recognize professional limitations and consult with supervising or collaborating physicians and other members of the healthcare team as appropriate. Coordinate referrals and higher-level care when necessary.
• Build Positive Relationships: Maintain professional, respectful relationships with patients, families, colleagues, healthcare providers, and members of the community.

• Strong critical-thinking and problem-solving skills to assess patient needs, initiate appropriate treatment, and determine when referrals or higher-level care are necessary.
• Excellent verbal, written, and listening skills, with a commitment to outstanding patient service.
• Understanding of patient experience and satisfaction measures, including Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS), and the ability to support positive patient experiences.
• Strong documentation, organizational, and time-management skills.
• Self-motivated, adaptable, flexible, and capable of making informed clinical decisions.
• Commitment to teamwork, continued professional development, and high-quality patient care.
• Willingness to perform additional duties as needed to support clinic and organizational operations.

Requirements

Education/Experience/Licensure

• Master's degree from an Accredited University
• Advanced Practice Provider, Certified Nurse Practitioner, or Physician Assistant license for South Dakota
• Certification in BLS, ACLS
• Current State and Federal Narcotics Licenses
• 3+ years of clinic experience
