We are seeking a compassionate and dedicated Part-Time Family Nurse Practitioner to provide high-quality primary care services at our Child & Adolescent Health Center (CAHC) located within TCAPS West Middle School in Traverse City, Michigan.
About the Role
The Family Nurse Practitioner provides comprehensive primary care services to students ages 3-21 in a school-based clinic, delivering patient-centered care through assessment, diagnosis, treatment, and follow-up. Working collaboratively with an interdisciplinary team, this role supports prevention, wellness, and continuity of care while helping students overcome barriers to accessing healthcare.

Key Responsibilities
- Provide comprehensive primary care services to students ages 3-21 in a school-based health center.
- Perform physical assessments, diagnose illnesses, develop treatment plans, and prescribe appropriate therapies.
- Conduct preventive health screenings and provide referrals for specialty or supportive services as needed.
- Coordinate follow-up care and collaborate with families, school personnel, and healthcare providers to ensure continuity of care.
- Document patient encounters accurately and maintain compliance with coding, HIPAA, FERPA, and organizational standards.
- Participate in interdisciplinary care planning, case reviews, and collaborative practice meetings.
- Support Child & Adolescent Health Center quality improvement initiatives and program requirements.
- Promote health education, disease prevention, and wellness through school and community outreach activities.
- Participate in on-call responsibilities and after-hours patient support as required.
- Contribute to clinical policy development, staff education, and team-based care initiatives.
Qualifications
Required:
- Master's Degree in Nursing (MSN) from an accredited program.
- Active Registered Nurse (RN) license in the State of Michigan.
- Active Family Nurse Practitioner (FNP) certification from an accredited national certifying body.
- Active Advanced Practice Registered Nurse (APRN) license in the State of Michigan.
- Current Basic Life Support (BLS) certification.
- Active DEA registration or eligibility to obtain.
- Ability to meet credentialing requirements for insurance and regulatory entities.
- Valid driver's license.
Preferred:
- Minimum of three (3) years of outpatient primary care experience.
- Experience working with pediatric, adolescent, or underserved populations.
- Knowledge of Federally Qualified Health Center (FQHC) and Child & Adolescent Health Center (CAHC) programs.
