Position Summary: Provides high quality health care services to patients with diabetes. Prescribes medications, procedures, and training programs in order to conserve, improve, or correct patient health. Serves as an organizational and community resource on the topic of diabetes treatment and prevention. Performs administrative and supervisory duties as required.

Critical Tasks:

  • Diagnoses patient conditions, prescribes or provides treatment, and arranges for specialized care as needed in order to provide high quality health care services to patients, specifically with diabetes.
  • Prescribes corrective procedures to conserve, improve, or correct patient health through the use of therapy, training, or prescribed medications.
  • Oversees other licensed and non-licensed health care providers within the clinic in the absence of the Life Center Medical Director in order to ensure continuity of leadership and effective delivery of care.
  • Educates and coordinates development programs for clinical providers, nurse practitioners, registered dietitians, nurses, and medical assistants in order to foster an effective and efficient workforce.
  • Contributes to the development of diabetes related informational resources to share with staff, patients, caregivers, families, and the greater community.
  • Documents patient conditions and medical histories in order to maintain accurate and relevant patient information.
  • Performs other job-related activities as requested.

Required Qualifications:

  • Master's Degree in Nursing, Family Medicine or a closely related subject; Doctoral degree in Nursing preferred.
  • Arizona licensed Nurse Practitioner in good standing
  • Registered Nurse Licensure in AZ or qualified Compact Licensure.
  • Current and valid DEA license for prescribing and dispensing medication.
  • 3 years of experience with diabetes focus required.
  • Current and continuing BLS certifications required. Demonstrates ability to relate to diverse cultures and specifically the Gila River Community and/or other Native American cultures and community health services.
