- Deliver primary care services to adult patients, including health screenings, diagnostic evaluations, and therapeutic interventions
- Perform detailed medical assessments and formulate individualized care plans
- Facilitate coordination of services and specialty referrals in partnership with clinical staff
- Complete thorough and timely medical records documentation and engage in periodic chart audits
- Work alongside clinical leadership to enhance care delivery processes and overall service excellence

- Masters Degree as a Nurse Practitioner
- 2 plus year experience as Nurse Practitioner
