Delta Health is seeking a compassionate and patient-focused Family Practice Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our growing primary care team at our West Elk clinic in Hotchkiss. This position offers the opportunity to provide comprehensive outpatient care to patients of all ages while enjoying the exceptional quality of life available on Colorado's Western Slope.

As a valued member of our healthcare team, you will work collaboratively with physicians, advanced practice providers, nursing staff, and care coordinators to deliver high-quality, patient-centered care to our community.
