Our team is currently searching for exceptional Nurse Practitioner talent for our Home Base/Home Assessment opportunity, located in Albuquerque, NM.
The Home Base / Home Assessment program delivers high quality medical care to chronically ill, elderly patients in the comfort of their own home. As part of a team of providers and support staff, the Nurse Practitioner will have enough time to evaluate the patient's needs and develop a proactive and preventive healthcare plan. The Nurse Practitioner is provided all the equipment and medical supplies needed to provide care in the home, and utilizes an electronic medical records system (EMR) to accurately and efficiently document their visits. The NP is responsible for establish and maintaining effective relationships with patients, key decision-makers, and collaborating physicians.
Nurse Practitioners with the desire to really make a difference in the lives of the patients will find this position very rewarding. This position requires travel to patients' homes and shared call responsibility.
Requirements

Active unencumbered license to practice nursing in the state

Master's degree required
-
NP Certification from either the ANCC or AANP
-
GNP, FNP, ANP specialty a plus
-
Minimum of 2 years experience
-
Superior communication skills
-
Comfortable working independently in the field

Basic knowledge of computers, fax machines, internet and cell phones

Familiarity with ICD-9 and CPT-4 coding, a plus
-
Bi-lingual or multi-lingual a plus

BCLS a plus
-
Excellent organizational skills with the ability to multi-task

Must have experience working within a team environment
