Nurse Practitioner
Matrix Medical Network provides high risk member management to health plans throughout the country. We are seeking full time and part time Nurse Practitioners to complete health risk assessments on plan members in their homes and in skilled nursing facilities throughout the State of Oregon. We are looking for full time in the Grants Pass and Medford area or an Inner State Traveler that can assist in coverage from Portland and SE areas to Medford.

POSITION SUMMARY:
The role of the Nurse Practitioner (NP) is to provide high-quality nursing care to meet the specialized needs of patients. Based on educational preparation and experience, NPs practice with a focus on a variety of specialty based populations including neonatal, pediatric, neurosurgery, cardiovascular, and adult. Besides clinical care, NPs focus on health promotion, disease prevention, health education and counseling. They help patients make wise health and lifestyle choices. The Nurse Practitioner works within the scope specified by the State Board of Nursing Scope of Practice Guidelines and the NA Scope of Practice statement for Nurse Practitioners, as well as the medical staff bylaws.
KEY RESPONSIBILITIES:
· Field based position completing health assessments for individuals in their homes and in Skilled Nursing Facilities
· Ensure the completed assessments are legible and accurate
· Identify patients that may meet disease management programs
· Maintain open lines of communication with an emphasis on supportive relationships with patients, and health plan staff
· Strong professional level of knowledge and comprehensive clinical assessment skills in the elderly population and chronic disease management
· Prior experience as a nurse practitioner in a long term care setting
· Strong decision making skills

QUALIFICATIONS: |
· Licensed as a registered nurse in the state of practice;
· Current nurse practitioner certification in specialty in the state of practice.
· Requires skill level typically achieved with 3-5 years of clinical expertise working with the specific patient population in the patient care area or clinic.
· Demonstrates proficiency in computer applications and database management.
· Requires ability to communicate effectively with others.
· Exhibits a professional demeanor and interpersonal skills.
Master's Degree in Nursing required
Three years expereince as an NP is required in the field of FNP with adult, geriatric and family specialities.
