HCR ManorCare provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy.Heartland Hospice Services of Newark DE has an immediate need for a Per Diem Nurse Practitioner.
The Per Diem Nurse Practitioner will provide medical assessments. Interventions will be performed for patients in the hospice and palliative setting. The NP will work as a member of the interdisciplinary team as applicable to effectively designate processes for meeting the medical, psychosocial and spiritual care needs of the patient. The NP will develop updates and evaluate medical treatment plans, medications, and other necessary care.

At least one year in Hospice/Palliative care Prior Nurse Practitioner experience, preferred Job Specific Details: Previous Hospice experience preferred.
Currently licensed as RN in state; Masters Degree in Nursing and certification through a national Nurse Practitioner licensing board.
