Kentucky Magnet Hospital is currently seeking an experienced APRN to develop and coordinate the NEW Advanced Heart Failure Clinic and Resource Center program. This is also a new position.
The APRN will assist in the management of patients to include but not limited to:

New patient admission
-
Daily rounding
-
Ongoing assessment and intervention
-
Coordination of inpatient and appropriate discharge planning to the Advanced Heart Failure Clinic.

Collaboration will be expected between referring physicians, cardiologists and other specialists.
To be considered, all candidates must have MSN, board certification as Acute Care or Family Nurse Practitioner and have a KY APRN license. Minimum of 3 years experience as an APRN managing patients in an outpatient or inpatient setting for patients with advanced heart failure is required and two to three year as RN with experience in cardiovascular ICU is preferred.
